  • Adjusted Clopidogrel Loading Doses According to Vasodilator-Stimulated Phosphoprotein Phosphorylation Index Decrease Rate of Major Adverse Cardiovascular Events in Patients With Clopidogrel Resistance: A Multicenter Randomized Prospective Study Laurent Bonello, Laurence Camoin-Jau, Stéphane Arques, Christian Boyer, Dimitri Panagides, Olivier Wittenberg, Marie-Claude Simeoni, Paul Barragan, Françoise Dignat-George, Franck Paganelli This study evaluates the clinical impact of adjusting the clopidogrel loading dose according to the vasodilator-stimulated phosphoprotein (VASP) index in patients with clopidogrel resistance (VASP index >50% after 600-mg loading dose) undergoing percutaneous coronary intervention. A total of 162 patients were randomized to a standard clopidogrel loading dose or adjusted loading dose to decrease the VASP index <50%. The VASP index was significantly decreased after loading-dose adjustment. There were significantly more major adverse cardiac events in the control patients but no difference in the bleeding rate. Adjusting the clopidogrel loading dose according to the VASP index is safe and significantly improves clinical outcome after percutaneous coronary intervention in patients with clopidogrel resistance.
