Serial Measurement of Monocyte Chemoattractant Protein-1 After Acute Coronary Syndromes: Results From the A to Z Trial James A. de Lemos, David A. Morrow, Michael A. Blazing, Petr Jarolim, Stephen D. Wiviott, Marc S. Sabatine, Robert M. Califf, Eugene Braunwald Serial measurements of monocyte chemoattractant protein (MCP)-1 were performed at baseline, 4 months, and 12 months in patients with ACS enrolled in the Z phase of the A to Z (Aggrastat to Zocor) trial. After adjusting for standard risk predictors, as well as levels of C-reactive protein and B-type natriuretic peptide, an elevated baseline or 4-month level of MCP-1 >238 pg/ml remained independently associated with mortality and with multiple composite end points. These findings suggest that MCP-1 provides independent prognostic value in the acute and chronic phases after acute coronary syndrome and merits further evaluation as a prognostic marker and potential therapeutic target.
