Définir la place de la prostatectomie totale dans la stratégie thérapeutique du cancer de la prostate localisé du patient âgé.
Il s’agit d’une revue non systématique de la littérature faisant le point sur les preuves scientifiques en lien avec la prostatectomie totale et le patient âgé.
Au total 17 publications ont été analysées et décrites afin d’établir une mise au point sur le sujet.
Les comorbidités et l’agressivité tumorale doivent être les principales variables justifiant une intervention chirurgicale. Les patients âgés présentent des cancers de prostate plus agressifs et sont souvent sous-traités. L’âge civil, sous réserve d’un bon état général, n’est donc pas une limite à la prostatectomie totale avec des résultats similaires en termes de contrôle carcinologique. Le risque d’incontinence prolongé après la chirurgie est acceptable mais toutefois supérieur à celui d’un patient plus jeune, un candidat à la prostatectomie totale devra donc en être informé.
To define the role of radical prostatectomy in the elderly therapeutic management of prostate cancer.
It is a non-systematic review of the literature, resuming scientific evidences on elderly radical prostatectomy.
A total of 17 publications were analyzed and described to clarify the topic.
Tumor aggressiveness and comorbidities should be the main variables to consider for a surgical procedure. Elderly patients present more aggressive prostate cancer and are often under treated. Chronological age, subject to good general condition, is therefore not a limit to radical prostatectomy with similar results in terms of cancer control. The risk of prolonged incontinence after surgery is acceptable, but higher than that of a younger patient; a candidate for total prostatectomy should therefore be informed.
