  • An increasing number of flow cytometry (FC) assays for primary immunodeficiency disorders (PID) are becoming available for routine use in diagnostic laboratories. FC methods allow a faster turnaround time than western blot and molecular methods. Although FC assays for PID may not always give a definitive diagnosis, a provisional diagnosis can often be provided, guiding clinical management and follow-on investigations. Many FC assays for PID can be performed on whole blood. Whole blood assays take less time to perform than separated cell assays, and require less blood from the patient. At RCH, equipment used for radioactive assays is aging and due for replacement, prompting the development of FC methods for assessing lymphocyte proliferation and natural killer cell function. An overview of FC assays for the evaluation of PID will be presented and FC alternatives to tritiated thymidine and chromium release methods will be reviewed. Practical aspects such as quality control, reference ranges and cost management will also be discussed. Establishing a comprehensive FC diagnostic service for PID has been rewarding. RCH patients with suspected severe PID can now be provided with a rapid provisional diagnosis, facilitating appropriate management of their specific PID and timely workup for haematopoietic stem cell transplantation.
